Fees & Scheduling

I do not currently take insurance. I accept cash, credit card and personal check. The fee is for a 50 minute session and is expected at session time. I am also able to bill third party payers if needed—with the exclusion of insurance companies. If there is a no show to a session or a late cancel without legitimate reasons, clients will be charged a half session fee.

I typically see clients on a weekly basis for optimal treatment progress (more or less often can always be negotiated as needed). I can be contacted by phone or e-mail listed on my contact page of the website. I will do my best to get back to you as soon as possible. If a client needs to cancel a session, 24 hours notice is appreciated.

In order to save time for the session, you can download the intake package paperwork here, print a copy, fill it out and bring it to the first session.

Individuals and Couples

Couples will need to fill out one form for each person (pgs. 1-5) and sign the last section together (pgs. 5-11).
